  5. A statement confirming that the material is used without authorization;
  6. A statement confirming the accuracy of the information provided.

Title 17 USC §512(f) allows for penalties against those who misrepresent information in a copyright infringement notification.

Counter Notification – Restoration of Material

If you believe your material was wrongly removed due to a copyright claim, you can send us a counter notification with your signature, details of the removed material, and a statement under penalty of perjury confirming your belief.
